Do I Qualify?

To be eligible for services at the Free Clinic of the Greater Menomonie Area, you must meet all of the following criteria:

  1. Income Eligibility
    Your gross household income must be at or below 250% of the Federal Poverty Level (FPL) (see guidelines below).
  2. Insurance Status
    You must be uninsured or economically underinsured.
  3. Benefit Eligibility
    If you are eligible for Medicare, Medicaid, or Veterans Affairs (VA) benefits, you are not eligible for Free Clinic services.
  4. Proof of Income
    You will be asked to provide proof of income for yourself and your spouse, if applicable. Acceptable documentation includes:

    • Federal income tax return
    • Recent pay stubs
    • Social Security benefit statement
    • Unemployment benefit statement
